    0520-12-01-.06 Program Operation

    On Friday, May 29, 2026, a three-year-old student was advertently left on the playground when the three-year-old and four-year-old classrooms returned to the building. The child was separated from the group for approximately one and one-half hours. During an interview with DCS, the child reported that he entered a building adjacent to the playground to use the restroom. Upon exiting the restroom, he discovered that his class was gone. 0520-12-01-.06 Program Operation. (1) Each program shall maintain a system that enables all children in the program's care to receive a level of supervision that is appropriate to their age and their development, physical and mental status so as to ensure their health and safety and that allows program personnel to know the whereabouts of each child. (c) Children three (3) years through five (5) years of age shall be safely protected by an adult in close proximity to and not distracted by other tasks and must be able to physically respond immediately. (i) Staff shall position themselves strategically for adequate supervision and quick response. (j) Staff shall be aware of the dynamics (Interactions) and activities of the entire group even when dealing with only part of the group at a time. (l) Teachers shall monitor children's toileting and be aware of their activities while respecting the privacy needs of the child. Programs must have a bathroom supervision plan to reflect the developmental needs of children in the classroom. Items in plan may include, but are not limited to, location of staff, times of bathroom breaks and locations, and emergency bathroom procedures. (m) The program shall maintain a plan, approved by the Department, that enables a teacher in an emergency situation to call a second (2nd) adult who can respond quickly while maintaining as much supervision of the children in care as is possible under the circumstances. — Visit: Non-CCDF,Complaint Investigation — Action taken: On July 1, 2026, I conducted a follow-up visit and met with the director, Shirley Eimmerman. The observed classroom ratios were 1 years 2:6, 2 years 2:8, 3 years 1:5 and 4 years 2:15 with the additional support of a Foster Grandparent. Children's toileting procedures were observed and monitored while respecting each child's privacy. Required adult-to-child ratios were maintained both indoors and on the playground. Staff were strategically positioned to ensure all areas of the playground were supervised. One staff member was stationed on the porch to monitor children as they entered and exited the White House restroom. Each classroom maintained a current classroom roster to account for the number of children present. Before leaving the playground, a staff member checked inside and then gave the "all clear" for the group to proceed to the building. Head counts were conducted before departing the playground. Staff were appropriately positioned during the transition, with one staff member at the front of the line, one in the middle, and two at the rear to ensure adequate supervision.

    On the dates and times of the visits, there was no documentation provided for the pre-service training for Teresa Heidel, who taught 5th grade during the school year but was assigned to the four-year-old class for the summer. 0520-12-010.07 Staff. (5) Prior to assuming duties, each new employee shall receive two (2) hours of orientation, and be able to explain the Program philosophy, emergency procedures, policies regarding discipline of children, policies regarding the reporting of child abuse, Sudden Infant Death Syndrome (SIDS), and policies for receiving and dismissing children. (6) Within the first two (2) weeks on the job and annually thereafter, each employee, including directors, teachers, substitutes, volunteers, and practicum students, shall receive annual instruction in: (a) Child abuse detection, reporting, and prevention; (b) Parent-center communication; (c) Disease control and health promotion; (d) An overview of certificate of approval requirements; (e) Information on risks of infection to female employees of childbearing age; (f) Food allergies; (g) Supervision during high-risk activities such as eating and outdoor play; (h) Meal service and safe food preparation policies; (i) Shaken baby syndrome; (j) Abusive head trauma; (k) Safe sleep procedures; (l) Developmentally appropriate practices; and (m) Policies regarding discipline and supervision of children. (7) All training shall be documented in the Program's records and available for review by the Department's staff at any time. (8) The Program must maintain written documentation that each employee has read the full set of all applicable rules. In addition, a copy of these rules shall be maintained in an area that is readily accessible to all staff. — Visit: Non-CCDF,Complaint Investigation — Action taken: On July 1, 2026, I conducted a follow-up visit and met with the director, Shirley Eimmerman. Staff files were reviewed, and the required documentation was provided. Documentation verified that the "Before You Begin" and supervision training has been provided to staff.

The director stated that required documentation will be provided to parents as necessary.

    On the day of the incident, the child was not present with his class during the scheduled lunch period. After the child was located and reunited with the class, a lunch meal was not provided to him. 0520-12-01-.11 Food. (1) If the program provides meals, the program provide developmentally appropriate meals, snacks, and drinks for each child that are sufficient proportions and nutritional value to meet each child's health needs in accordance with the following minimum requirements: (b) Children in care five (5) to six (6) hours shall be provided one (1) meal and one (1) or two (2) snacks. However, if the child is fed their meal at home or in school, the child shall be served two (2) snacks in lieu of a meal. (c) Children in care seven (7) to ten (10) hours shall be provided one (1) meal and one (1) or two (2) snacks. (i) Food shall not be forced or withheld from children. — Visit: Non-CCDF,Complaint Investigation — Action taken: On July 1, 2026, I conducted a follow-up visit and met with the director, Shirley Eimmerman. During the visit, children were observed participating in playground activities, large-group instruction, free-play activities, and lunch. Children washed hands before lunch. The meal included a choice of peanut butter and jelly sandwich or a turkey sandwich, cucumbers, pickle, and orange, and milk. Children ate in the dining area with adult supervision.

    0520-12-01-.06 Program Operation, (4) Programs must meet the following group size and adult:child ratios: Three (3) years old 1:9. During the visit the ratio for the 3 year old class was 1:13. — Visit: Non-CCDF — Action taken: On February 20, 2026, I conducted a follow-up visit to reassess classroom ratios. I met with the director, Shirley Eimmerman, and observed the classroom ratios that were 1 year 2:8, 2 years 2:8, 2 years 1:7, 3 years 1:6, 3 years 1:7, and 4 years 1:10. The corrective action plan had been submitted, indicating that the preschool will maintain a substitute teacher list for emergency situations similar to the one previously identified. The program has also implemented a new policy requiring staff to communicate their classroom role to the director if she is working in a classroom and unable to take in-person roll. This ensures that an available assistant can step in immediately during an emergency. I reviewed the substitute list during the visit. The program is currently in compliance. No additional follow-up visit is required.
