1240-04-01-.11 Supervision
LC recieved a self reported incident stating a child had escaped through the outside playgroud gate. The video showed the child was outside the gate for 24 seconds before the educator brought the child back inside. The child squeezed through the locked gate. The child did not enter the parking lot. — Visit: Incident or Injury Investigation — Action taken: Agency has purchased a chain to secure the gate more tightly. They will add additional materials to completely close the gap between the fence and gate. Agency will complete Playground Supervision with CCR&R.
