    During a QRIS visit it was found that in the wise owls and the mighty chipmunks classrooms there is expired medication. These medications will be sent home and the parents notified to bring in new medications. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The agency as take the action of notifing the parents of two children who had expired medication on the afternoon of April 28, 2026 that their child could not return to school until they provided non-expired life saving medication. Both have provided non-expired medication as of 4/30/2026 and have returned to school. Going forward the agency plans to add Marion Anthony-Law to Medication Expiration Dates Document Recheck all expiration dates on emergency medications and confirm accuracy on Medication Expiration Dates Document Check SGS office calendar for parent reminders (to be sent to parents the month before medication expiration date) Add expiration dates for each child's rescue medication to SGS office calendar (last day of the month of expiration) Add date of monthly emergency backpack/expiration date check to SGS office calendar Conduct monthly emergency backpack checks, including expiration dates Track monthly emergency backpack/expiration check in sharepoint document (SGS Health Information folder) Communicate with families about medication via email and copy sgsoffice@vanderbilt.edu (can message in Brightwheel that we have sent an important email regarding medication) Children may not attend school if rescue medication is expired

    During QRIS monitoring and serious injury investigation it was discovered that age child was in a swing that was not developmentally appropriate for his age. This caused the child to fall out of the swing resulting in injury. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The equipment was repaired and the provider has trained the staff on reporting damanged equipmet. There has been a plan put in place with the physcial therapist that educators review the list for children at what equipment each child can use.
