In review cof the Kinder Care App, there was numerous messages sent by a parent since 2/2026- current with no responses documented. There is no written verification that staff responded to the parent. The agency did nothave verification of answering parent questions or concerns. — Visit: Technical Assistance,Complaint Investigation — Action taken: The agency will communicate with families using the Kinder Care App and document conversations and concerns. The director will respond to parent/guardians in a timely manner.

1240-04-01-.11 Supervision

A blanket was observed in a crib with a sleeping infant. To maintain safe sleep requirements, blankets are prohibited. — Visit: Technical Assistance,Safety Plan Monitoring — Action taken: The agency director completed staff training in SIDS the same day the violation was cited. The assistant director will be responsible for monitoring the infant classrooms throughout the day to ensure safe sleep procedures are being followed.

1240-04-01-.10 Duty to Report Child Abuse and Neglect

During an complaint investigation it was determined an educator did not report allegations to the Department of Children's Services as required. — Visit: Follow-up visit,Safety Plan Monitoring — Action taken: Provide retraining to all staff on reporting requirements, timelines, and documentation procedures in accordance with agency policy and licensing regulations. Review reporting policies during staff meetings to reinforce expectations and clarify when and how incidents must be reported. Ensure reporting procedures and required forms are readily accessible to all staff at all times. Conduct periodic audits of incident reports to ensure compliance with agency policies and licensing standards. Implement additional supervision or check-ins with staff as needed to support compliance and accountability.

An infant was mistakely given another child's bottle. — Visit: Technical Assistance,Incident or Injury Investigation — Action taken: Educators were retrained on infant room procedures. Management will be observing in the classroom more often to ensure that name to face steps for feedings are being followed.

A parent was not notified the same day as an incident. — Visit: Technical Assistance,Incident or Injury Investigation — Action taken: The director has retrained all educators on infant feeding procedures and following the agency policy of name to face feedings. All agency staff have been trained on reporting incidents to parents on the same day of the incident.
