During an unannounced complaint and probation monitoring visit on 05/06/2025, Licensing Consultant and Field Supervisor noticed that children's files had incident/accident reports that were not completed and also parents were not notified the day the incidents occurred. During an unannounced complaint and probation monitoring visit on 05/06/2025, Licensing Consultant and Field Supervisor noticed that the receiving educators in the one-year-old class or the Pre-K class did not observe each child upon their arrival for signs of illness and injury. An educator stated that she does not check the children every day. During an unannounced safety plan monitoring visit on 02/27/2025, the Licensing Consultant observed staff in the infant room who had not been cleared by the criminal background department before having access to children. — Visit: Safety Plan Monitoring — Action taken: Based on the agency, staff will not be allowed to work until they have been cleared to work by criminal background.

During an unannounced safety plan monitoring visit on 2/27/2025, the Licensing Consultant observed the ratio was not compliant due to a staff member who was present, could not be counted in the ratio because she had not been cleared to work by the criminal background department. — Visit: Safety Plan Monitoring — Action taken: According to the agency, staff will not be able to work until their background have been cleared to work.

1240-04-01-.22 Specific Requirements for Child Care Centers

During an unnanounnced visit to the agency to serve a safety plan. The Licensing Consultant (LC) observed the Infant room (ages 3 mo-1 yr old) was over capacity and out of ratio (2:9). The 2 yr old room and the pre-K room (ages 2-4) were combined for breakfast but were also out of ratio and overcapacity (2:25). When the LC told the Infant educators that they were out of ratio, an Educator who was preparing breakfast (in the kitchen) took one of the 1-year-olds out of the infant room, to the pre-k room. Now there were 2 educators and 26 children present (2:26), and they were 1-4 years of age. When the director arrived to the agency, the LC was in the Prek room where the 1-4 yr olds were combined. The LC was walking back to the front of the agency with the director, when the LC observed one educator in the classroom with 8 infants by herself. One of the educator's left the room and was in the kitchen across the hall getting milk. The LC told both educators at no time are they ever to leave the classroom with one educator at 8 infants and the ratio of 1:4 shall always be maintained. The educator returned to the classroom. — Visit: Safety Plan Served — Action taken: The agency will ensure that 2 educators are present at all times when there are 8 infants in the room.

During an unannounced visit, the Licensing Consultant observed the educator not washing the children's hands after diapering, not properly utilizing the sanitary or diapering procedures and not washing her hands after diapering the children. — Visit: Safety Plan Served — Action taken: Per the director, the educator was spoken to regarding the importance of proper handwashing after diapering to prevent the spread of germs.

During an unannounced complaint visit conducted by Licensing Consultant Angela Thacker and Field Supervisor Evangelina Clear, during staff interviews, it was said that there were buckets used to wash children's hands after breakfast. Also, staff stated that they only use soap and water to clean the high chairs before eating and bleach and water used after they eat. — Visit: Follow-up visit,Complaint Investigation — Action taken: Per the agency, the children are taken to the sink to wash their hands before and after eating. Educators will also clean the tables with soap and water, then followed by bleach and water prior to eating and after eating.

During an announced visit on 01/28/2025, the licensing consultant observed two educators who have been employed for over 90 days had not completed their first aid and CPR trainings. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The educators will take the training online and they are signed up for a training on February 2, 2025, as well.
