1240-04-01-.11 Supervision

A child, age 5years old, was left in the classroom sleeping on her mat during a fire drill. It is recorded that the child was left alone for approximately two minutes in the classroom before the Director found the child during the fire drill sweep. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The staff involved in the incident has been terminated, and the parents of the child was notified via a conversation with the Director and a formal incident report. The Agency also self-reported to DHS via phone call and the Provider Portal. A mandatory staff meeting will be held to review the Emergency Preparedness Plan (EPP), name-to-face counting, and leadership will conduct ongoing observations to ensure safety protocols are being met, and monthly fire drills will continue as required.

It is recorded that the child was left alone for approximately two minutes in the classroom before the Director found the child during the fire drill sweep. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: All staff have been retrained on active supervision, name-to-face attendance checks, and emergency procedures. Fire drill protocols are clarified so that no child is left behind under any circumstance. The staff member involved was terminated, the child's parents were notified, and the agency self-reported to DHS as well as DCS.

1240-04-01-.16 Physical Facilities

During a Serious injury/ incident Investigation, LC Champion and Carter were informed that a pre-playground inspection was not conducted prior to the Preschool 3/4 classrooms going out onto the playground. During outside time, one of the educators noticed that there were pieces of glass on the playground and began to pick them up at that time. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The Agency will continue doing pre-playground inspections a the opening of school and will begin conducting pre-playground inspections before each classroom playground designated time. The Agency will also train staff on the proper pre-playground inspection protocols and begin documenting all preplay inspections. The Agency is also reviewing the playground structure/ layout, as rocks are their main layout. Continuous monitoring of preplay inspections has been put in place to ensure the Agency is providing the children a safe environment to engage in play.

The agency failed to report a serious injury/ incident to TDHS promptly. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action: Staff are being reminded of the importance of and TN Rules and Regulations regarding timely incident reporting. As well as a parent follow-up protocol is being put in place to ensure reporting is being done promptly and appropriately, and the agency staff are aware of the child's condition following any injury/incident.
