During an unannounced visit to investigate an incident, after speaking with educators, director and watching video of the outside area of the school it was determined that a child was left unattended outside the school for 7 minutes. A Name to Face Roll Call was not done once arriving inside the building. — Visit: Incident or Injury Investigation — Action taken: Staff involved have been put on leave and at this time retraining the staff in NTF with another coworker in supervision, roll call and taking attendance through the transition of the day. Making eye contact with each child when doing the name to face and counting children when leaving any area in the building and outside. Assist the staff in training and make sure that she knows each child in her group,have to do NTF throughout the day with the children.

And keeping roaster current for each child during arrival and departure. Retrain and hold staff accountable, on making they under the rules and the importance of name to face.

During an unannounced QRIS visit LC also investigated a serious injury incident. After LC spoke by phone to the child's parent it was determined that the parent was not made aware of the child's fall when he was picked up from Fun Company. — Visit: QRIS Monitoring visit,Incident or Injury Investigation — Action taken: I have retrained all staff on how to complete the proper paperwork, completing Ouch reports for Parent and Incident Reports for YMCA SAS. Retraining staff on what to do if a child gets hurt. Staff notify Site Director and Notify Parents by phone and arrival. Also letting Staff know that Ouch/Incident reports must be completed at all times when there is accident involving children.
