On 4/28/2026, the school conducted a monthly fire drill during rest time. The educators called face to name from memory; not an accurate roster. As a result, a three year old child was left unattended while she slept on her cot for approximately 7 minutes. After a lapse of time, the educators realized that the child was missing and notified the administration. Administration immediately went inside and located the child who was still asleep on her cot in the classroom. — Visit: Complaint Investigation,Safety Plan Served — Action taken: Read and reviewed.

The agency self-reported on 04/03/25 that an educator left a child on the smaller playground while transitioning to the big playground. LC Bria Woods found out about the incident by email just minutes after it happened. LC Bria Woods then told the agency to report it in the system. LC Bria Woods completed an unannounced visit on 04/07/25.
