Nine (9) children were present, but only 7 children were signed into the agency. One child had the time written for arrival, but not the name or signature of whoever dropped off. One child was not signed in at all. — Visit: Other,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): The agency was able to get the child's parent's name added to the sign-in sheet by the director, with a notation to have the parent correct it upon dismissal. The other child was added to the sheet, the parent was notified of the error, and the sign-in sheet was corrected. At this time, the agency is coming up with a plan to keep labeled sheets on hand at all times and to assign an educator on all shifts to be responsible for the accuracy of the sign-in sheets in case anything like that ever happens again.

The provider made it a rule that 2 through 5-year-olds sleep in one room unless there is adequate staff coverage to split them up. The provider's goal is to hire additional staff to support the current educators and ensure that there are no children left unsupervised. Additionally, the director will frequently train and monitor the supervision to ensure this rule is being adhered to by all educators moving forward.

During an unannounced visit on August 20, 2025, a supervision lapse occured. — Visit: Follow-up visit,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): The provider moved the 11-month-old to her pack-and-play for naptime. The provider will have moved all 2-5-year-olds to one room for naptime. That will leave one educator free to help clean and change infants and put them down for a nap, which will ensure there will be no gap in supervision during any transition. The provider will also consult with CCR&R for the best solution to this problem in case of staff shortages.

Moving forward we will move all bigger kids to one room and we will supervise them with one floater in rotation so that each educator will have a break. And they will be more sure to stay awake.

The agency said they will receive TA from CCR&R on being more aware of their surroundings and reading labels to keep all necessary things out of children's reach. The agency has established that one staff member will do frequent sweeps of the facility to ensure that no items are left in reach of children. All staff have agreed to hold each other accountable and support each other in this effort to keep kids safe in our care.

The director informed educators that as soon as an infant falls asleep, it is to be moved to a pack-and-play. Further staff have been trained on the safe sleep policy and will sign off on it. The provider is looking to hire more staff to support our infant educator.
