During an unannounced visit to investigate a serious incident complaint received on the All Day Out Program, after LC interviewed educators it was determined, that there was a lack of Supervision on the playground which resulted in inappropriate touch behaviors by two children. — Visit: Incident or Injury Investigation — Action taken: All parties involved parents were notified. The two children who took part in the inappropriate behavior has been suspended from the program until further notice. Staff present will undergo extra trainings on supervision/playground supervision.Staff will ensure that children are being properly supervised by actively walking and interacting with participants in the program. We will also ensure that staff placement in certain areas is appropriate and that all staff should be engaging with participants.

During an unannounced incident complaint visit on 12/26 there were no paper copies of incident reports for LC to review. Parents of the children involved in the incident and other children present were verbally told of the incident. LC was told there were paper Incident reports completed but have not been emailed to LC as of 1/8/25. — Visit: Incident or Injury Investigation — Action taken: Moving forward we will make sure to have proper documentations available on site. We will make sure staff document all incidents in a timely manner.
