71-3-508. Inspection of Entities Providing Child Care

The agency has been previously notified on multiple occasions regarding deficiencies preventing approval of the required state fire inspection. To date, the necessary correction has not been completed, and an approved fire inspection has not been obtained. This places the agency in continued non-compliance. — Visit: QRIS Monitoring visit — Action taken: Going to get it done and stay on top of the school. Get everything in by the due date.

1240-04-01-.08 Record Keeping

During an unannounced QRIS visit when LC was reviewing new staff files it was determined that there was a staff person working at the site without a file. — Visit: QRIS Monitoring visit — Action taken: Going to get a file for her Make sure to have one on site at all times.

1240-04-01-.06 Staff

There was 1 staff that didn't complete their annual and block grant trainings before September 30, 2025. — Visit: QRIS Monitoring visit — Action taken: Talk to my staff about to inform them to make sure the stay on top of their trainings and to be more intentional in getting them done. I will talk to and get them there training papers in a timely manner so they can have them done on time.

1240-04-01-.11 Supervision

During an unannounced visit to investigate a serious incident complaint received on the All Day Out Program, after LC interviewed educators it was determined, that there was a lack of Supervision on the playground which resulted in inappropriate touch behaviors by two children. — Visit: Incident or Injury Investigation — Action taken: All parties involved parents were notified. The two children who took part in the inappropriate behavior has been suspended from the program until further notice. Staff present will undergo extra trainings on supervision/playground supervision.Staff will ensure that children are being properly supervised by actively walking and interacting with participants in the program. We will also ensure that staff placement in certain areas is appropriate and that all staff should be engaging with participants.

1240-04-01-.09 Incident Reporting

During an unannounced incident complaint visit on 12/26 there were no paper copies of incident reports for LC to review. Parents of the children involved in the incident and other children present were verbally told of the incident. LC was told there were paper Incident reports completed but have not been emailed to LC as of 1/8/25. — Visit: Incident or Injury Investigation — Action taken: Moving forward we will make sure to have proper documentations available on site. We will make sure staff document all incidents in a timely manner.
