After conducting an unannounced visit on 04/22/26, LC Walker and Sanders determined on 4/20/2026 there was one educator to 11 children in the 2-3 year old room with 8 children under 30 months. — Visit: Complaint Investigation,Incident or Injury Investigation,Technical Assistance — Action taken: The agency will always schedule enough staff so the 2 year old classroom will never be out of ratio again during naptime.

When a classroom came in from the playground the educators did not conduct name to face roll cal when entering the classroom. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: All staff are being monitored and checked daily by management until further notice to ensure that the proper protocols are being followed with face to name count. They have all signed off that there are 6 stopping points, 3 going out and 3 going in, when having outdoor play or leaving a classroom. The agency will be receiving CCR&R training on March 12th for our professional development.
