    An incident occurred while the children were playing on the playground; one child picked up and threw an empty coffee can at another child. The coffee can is used as an exploration tool for children to fill and empty with mulch while playing. The child who was hit by the coffee can suffered a scratch on his head. The center did not provide an incident report to the parents in this case. — Visit: Complaint Investigation — Action taken: The Director will ensure that all staff are reminded that all incidents and accidents must be reported to management, documented for the child's file, and parents must sign the documentation the same day. Serious injuries must also be reported to the Department.
