During the serious incident investigation, LC checked the children's files for an official incident report for each child involved in the incident. No incident report was completed that meets the DHS Rules and Regulations requirements; however, the Director did have a document of an email that was sent to the Parents of the children involved, and an in-person meeting was held. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The Center will print a hard copy of all incident reports for the parents to sign and retain a copy in the child's file.

1240-04-01-.11 Supervision
The Agency self-reported that two children, aged 17 months and 22 months, walked out of the playground gate and down the sidewalk to the front of the childcare center on Tuesday, September 2, 2025. The educator in charge of their care was unaware that the children were gone and left unsupervised. The Director saw the children in front of the building and immediately retrieved the children and brought them safely back into the childcare center. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Equally Created has placed the educator involved on a 60-day probationary plan, all staff have reviewed supervision practices, each staff member has been given their own carabiner clip to place on the gate during playground time, and there will be two staff members on the playground during every playground time.
