The Educator(1) on duty left the children with access to food/eating to gather more food items from the kitchen. The relieving Educator (2) on duty, left the children with access to food/eating to assist a child in the restroom. During each occurrence, the educator on duty was the sole adult who was responsible for mealtime supervision, which is a high-risk activity due to choking/etc. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Agency provided with supportive Mealtime Supervsion materials and guidance. TAM Referral made to support the Agency in updating mealtime supervision plan and practices.
