A parent has not been properly notified on an incident that occurred with her child on 2/12/2026. — Visit: Follow-up visit,Complaint Investigation,Safety Plan Served,Technical Assistance — Action taken: An incident form was completed to properly notify parent and staff will be retrained on incident reporting.

It was mentioned that a child was left unsupervised in a classroom, but was not reported to DHS Child and Adult Care Licensure. — Visit: Complaint Investigation,Safety Plan Monitoring,Technical Assistance — Action taken: The agency was cited with violations. Disciplinary action was taken against the lead Educator. Both Educators present during the event were retrained in the transition procedure. All agency staff will be retained in supervision and name to face roll call on 4/16/26.

1240-04-01-.15 Program Language and Literacy Development

It was reported that an Educator has been witnessed "taunting" a child and using negative reinforcement. — Visit: Complaint Investigation,Safety Plan Monitoring,Technical Assistance — Action taken: The agency received a violation for behavior management and will be referred to an outside agency by LC Goss for training to help Educators deal with behaviors.

The center staff will now make a verbal confirmation that Central Office receives reports that are sent from the centers.
