   On 2-20-26, an unannounced visit to investigate a complaint was conducted by LC Macey Johnson (DHS) and Emily Rice (DCS), video footage from 2-10-26 was viewed. Educators Jamie Basham and Briley Payton were educators in the 6 weeks-30 month classroom. There was video evidence of both educators on their phones while children were eating and while feeding an infant a bottle. — Visit: Complaint Investigation,Technical Assistance — Action taken: Licensing and management have spoken with all staff. Training is being completed by all staff.

   On 2-20-26, an unannounced visit to investigate a complaint was conducted by LC Macey Johnson (DHS) and Emily Rice (DCS), and video footage from 2-10-26 was viewed. Interviews were conducted with staff from the agency; two educators stated that they had witnessed what they described as possible abuse and/or neglect with infants and toddlers in the 6-week-30-month room on occasions in January and February. The staff did not report their suspicion to the Department of Children's Services, the Department of Human Services, or law enforcement immediately as required. One staff member reported this information to the Department of Children's Services in mid-February. — Visit: Complaint Investigation,Technical Assistance — Action taken: All staff will complete training.

The educators in the 6 weeks-30-month classroom were observed placing an infant in a bouncy seat, and another infant was placed in a swing. Both infants were placed in the swing and bouncer for over an hour, and there were instances when the staff was across the room, leaving the infants unattended while seated in these devices. — Visit: Complaint Investigation,Technical Assistance — Action taken: All staff are being trained on infant care and supervision rules.

   On 2-20-26, an unannounced visit to investigate a complaint was conducted by LC Macey Johnson (DHS) and Emily Rice (DCS), video footage from 2-10-26 was viewed. Jamie Basham was an educator in the 6 weeks-30-month classroom. Jamie Basham was observed inappropriately pulling a toddler's arm aggressively. There were instances of a toddler throwing their milk cup on the floor and sliding out of the chair. Video evidence shows Jamie Basham picked the toddler up out of the chair inappropriately and aggressively placed the toddler back in the chair with force. In addition, she poked the toddler on the head with her finger, and she was observed holding the toddler's head while speaking close to his face when she was visibly frustrated. Jamie Bashem was also observed seated on the rocking chair holding an infant, she was patting the infant and bouncing her leg up and down, attempting to soothe the child. The video evidence shows that she gets up from the rocking chair and is holding the infant inappropriately and not supporting the infant's head while walking to another area of the room. She places the infant on a rug on the floor; at no time during this transition did she support the infant's head as needed. After placing the infant on the rug, she then can be seen on camera throwing her arms up in the air, shaking her fist, and was visibly frustrated while walking away from the infant. — Visit: Complaint Investigation,Technical Assistance — Action taken: Employee no longer works at the agency. Training will be completed by all staff.
