An educator in charge of supervising mealtime was reading a book to children seated at the tables while three (3) children were still eating. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The agency will provide an In-Service for Mealtime Supervision for all staff on 12/12/25. Added a sign with Big Bold Letters asking, "ARE EYES ON KIDDOS DURING MEALTIME?" ail the area where meals are served on 12/11/2025.

At an unannounced complaint visit on 5/13/2025, the Licensing Consultant observed the Director leave the Butterflies classroom unsupervised at 6:29 am for approximately 15-20 seconds as he went to the front desk to escort a child back to the classroom during early morning drop-off. The Director was within direct sight-line of the classroom but could not see the entire classroom at a glance when in the hallway. — Visit: Complaint Investigation,Technical Assistance — Action taken: A Placard was created, requesting that parents take their little ones to the class in the early morning to prevent similar action. Additional staff members will be coming in earlier.

An agency staff member in charge of reviewing transportation did not have evidence of completing the required DHS Transportation Training. — Visit: QRIS Monitoring visit,Transportation,Technical Assistance — Action taken: Any staff member that does not have the required trainings will not participate in transportation related activities such as reviews until they get the proper training. The agency has set up trainings to get more member of staff qualified. The ProSolutions training will be completed as soon as possible and staff are registered for the next DHS available training on 10/15/24.
