At an injury investigation visit it was determined that the children were praticipating in soccer shots in the gym and did not have an educator supervising that had a final clearance letter for the agency. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Going foward the agency plans to assign educators to supervise all outside vendors unless said vendors have the required DHS background check. The director plans to review the schedule each week to confirm educators can be placed. If educators are not available, the assistant director or director will step in.

When trainings were reviewed during a QRIS LC saw that educators didn't complete the Block grant trainings by 9/30/25. — Visit: QRIS Monitoring visit — Action taken: The administration will quickly compile the necessary trainings needed for each staff member and request they are completed by December 15 .The administration staff will make sure that each staff member's trainings are reviewed quaterly in TNPAL to make sure they are keeping up on trainings. We will also seek out information and ask questions pertaining to trainings or any changes that are communicated through our LC.

During an announced QRIS visit, while LC was checking medications in the classrooms, LC looked at one epi- pen it was found without a completed parent permission form and was expired. LC looked at another epi-pen and found that it was expired. — Visit: QRIS Monitoring visit — Action taken: Director has called the parents to inform them the epi pens were expired and that the permission form needs to be filled out. The agency will include expiration dates for medications on the allergy master sheet. The agency will also post expiration dates inside the medication cabinets so teachers and staff can see.

Staff training hours were not completed in the 2023-2024 QRIS year. — Visit: QRIS Monitoring visit — Action taken: The Director will make a better effort to make sure that she and other staff members complete the required hours. The Office Administrator will monitor the TNPAL account and notify staff quarterly regarding the number of hours they have. Also, the agecny will provide more onsite trainings.
