The agency was not in compliance with playground supervision as proper roll call was not conducted. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The agency is requiring all classrooms to contact the front desk so that a member of management can observe them completing NTF as they go outside as well as returning inside to ensure all children are accounted for. All staff are being re-trained on 11/18/2025 with CCR & R to cover Name to Face and supervision. Management will observe teachers completing NTF to ensure all classrooms are aware of how to do it as well as when to do it.
