Staff didn't complete all the required block grant or annual training hours by September 30th. — Visit: QRIS Monitoring visit — Action taken: A meeting was held on 11/26/2025, phone call between the Daycare Board Chairman and the outgoing Director to discuss the recurring issue of missing documents from the files. A decision was made that, for accountability purposes, only the Director on site will have a key to the file cabinet. Should potential clients/ staff come to receive applications before the Director arrives application packets will be available on the Director's desk. A staff meeting was held on 11/28/2005 to review the violation which occurred on 11/25/2005. All staff were present and accounted for. The Director discussed the plan to correct the violation and to combat the issue from recurring in the future. Each staff member was informed of the individual classes and number of classes needed that are necessary to rectify the issue. The Director formed a plan to relieve each staff member during their work schedule for 1-2 hours to take the classes on site.

During an announced QRIS visit LC reviewed staff files and found there were 3 staff files with no education records in the files. There was also 2 staff files with no physicals in the files. These educators have been working longer than 10 days. — Visit: QRIS Monitoring visit — Action taken: Staff were informed about the missing educational records. They will accept the TNPal invitation to join our center and send their High School Diploma's over to the Daycare via email today. Little Beginnings Daycare will ensure that all paperwork kept in files are locked and are out of reach of anyone when the Director is out of the building. We will also make sure all Educational documentation and trainings are kept in files. We will make sure that upon enrollment all documentation is given to the Director prior to start date.

1240-04-01-.11 Supervision
An educator was observed sitting outside the classroom door for a period of less than two minutes leaving the class unattended during naptime. A CCR&R coach conducted a visit on 1-3-25 at 1:00 PM. Upon arrival, one educator greeted the representative, facilitated entry, and requested that the visitor sign the log. Another educator was observed sitting in the hallway eating lunch, and the Director joined shortly after. It was further clarified that the teacher assigned to the classroom had stepped out momentarily to retrieve a plate of food. The Director acknowledged the lapse in supervision and indicated that the plate should have been delivered to the classroom to maintain proper ratio. Per licensing rules: At least one (1) adult educator shall be awake and supervising the children in each nap room/sleeping area; — Visit: Complaint Investigation,Technical Assistance — Action taken: A staff meeting was held to address the Safe Sleep policy violation that occurred on January 2, 2025. The staff member involved expressed remorse, acknowledging unawareness that sitting in the hallway with the door open was a violation. After reviewing the policy, the individual now understands the importance of adhering to Safe Sleep protocols. Director reviewed the Safe Sleep policy and procedures with all staff members. To prevent future incidents, a staff mealtime plan and schedule have been implemented. Additionally, steps are being taken to hire and train a new staff member to assist with meal preparation and provide coverage during naptime. This will ensure that staff can take uninterrupted lunch breaks while maintaining a safe environment for the children.
