    A renewal inspection was conducted on 5/13/26. Staff person #1 did not receive emergency plan training annually. The last emergency plan training on file is dated 10/30/24. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Correction: Staff person #1 had emergency plan training and signed our form that she did complete the training prior to coming back to the classroom on 11/04/25. Inspection: Renewal.     An unannounced inspection was conducted on 2/28/25. Staff #1 was observed providing childcare (see code sheet for DOH). The staff file contains a Bachelor of Art degree but does not contain college transcripts or documentation of childcare experience. The director reported that staff #1 is a group supervisor. Requirement: A group supervisor shall have a bachelor's degree from an accredited college or university, including 30 credit hours in early childhood education, child development, special education, elementary education or the human services field and 1 year of experience with children. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: Staff #1 now has a copy of his college transcripts and the childcare employment verification form on file. Staff #1 will now be listed as an assistant group supervisor.
