Observed in the indoor play space a small plastic sliding board on a tile floor lacking a protective surface covering. Requirement: Indoor play equipment for climbing shall be installed or used over a protective surface covering which does not interfere with the stability of the equipment. Correction: A carpet was placed in the gym storage area as of 26 Aug 2025. The carpet is now used under the slide when the slide is in use. Inspection: Renewal. Correction: As of 26 Aug 2025, Staff person #6 was placed on leave of absence. Until such time as the required pre- service trainings are completed, staff person #6 must be supervised, when interacting with children at a minimum by, an (AGS, primary staff person, or family child care home staff person) who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #6, staff person #6 may not work in a child-care position at the facility. Observed in the Toddler Room a bottle of Children's Tylenol labeled with the name of all 3 children in the family. Medication must be labeled and intended for one individual child. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: Parent identified which child's name would go on the bottle of Tylenol and bringing two more bottles for the two other children which will be labeled individually. Inspection: Renewal. Based on conversation with staff, it was verified that, on 8/26/24 at approximately 12:30 PM, Staff Person #1 roughly placed Child #1 on a mat at naptime. The child body lifted up off the ground and landed on side. Staff Person #1 was observed to sweep the foot of Child #1 causing the child to fall to the mat. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Owner addressed Staff Person #1's actions the day it happened. Based on conversation with staff, on 8/27/24, Staff Person #2 was on a speaker phone in the office discussing confidential information regarding two children at the facility. A parent had entered the office, unnoticed by Staff Person #2, who continued the conversation for approximately ten seconds before noticing the parent was in the office. Requirement: A facility person may not disclose information concerning a child or family, except in the course of inspections and investigations by agents of the Department. Correction: Owner verbally addressed Staff Person #2's actions on 8/28/024. Staff Person #2 acknowledged and understood the breach of confidentiality.
