Upon investigation, it was confirmed that Staff Person #1 had used physical force directed upon Child #1 in such a manner as to cause the child to fall onto their backside [no injury found]. Staff Person #1 was immediately terminated from employment as a result of this incident. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: WCC Management immediately investigated the incident, then immediately terminated employment of Staff Person #1. Management immediately self-reported the incident to their DHS representative. Staff will not use physical discipline/force on children. Inspection: Self-Reported Non Compliance. As a result of this investigation, it was found that Staff Person #1 responded to Child #1 normal childhood related behaviors [i.e., close proximity to adult] with physical force, rather than methods that could have promoted additional age-appropriate skill development and social competence with regards to adult:child interactions. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: WCC Management immediately investigated the incident, then immediately terminated employment of Staff Person #1. Availability of certificate of compliance and applicable regulations (3270.25(a))

Upon inspection of the required documents posted on site, it was found that the Certificate of Compliance in evidence was an expired copy. The facility took immediate steps to locate the current copy and placed it in a conspicuous location during the inspection. Requirement: The facility's current certificate of compliance shall be posted in a conspicuous location used by parents. The operator shall provide the parent of each child enrolled with information on how to access the regulations in this chapter electronically and with instructions for contacting the appropriate regional child care office. Correction: Within 10 minutes of the addressed violations, WCC replaced the former Certificate of Compliance with the current one [on the parent board] Inspection: Renewal. Glass (3270.81)

During inspection of each childcare space, it was found that glass located in the 'Upstairs Tables' space did not have a visual strip or other means of visual identification. The facility took immediate steps to place the required identification markings into place during the inspection. Requirement: A visual strip or other visual identification shall be placed on glass located in a traffic area, a child care space or a play space. Inaccessible to children (3270.75(b))

The first aid kit in the "Little Room" was accessible to children in an unlocked cabinet at ground level. (Corrected during inspection.) Requirement: A first-aid kit must be inaccessible to children. The clearances for Staff #1 and #2 were not updated every 60 months as required. The file for Staff #1 included NSOR verifications dated 1/27/2020 with an update on 1/30/2025. The file for Staff #2 included state police clearances dated 1/6/2020 updated 1/10/2025, child abuse clearances dated 1/10/2020 updated 1/16/2025, NSOR verifications dated 1/14/2020 updated 1/24/2025, and mandated reporter training dated 1/5/2020 updated 1/10/2025. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). It was observed that fire drills were not conducted at least once every 60 days. The facility's fire drill log included drill dates on 5.1.24 and again on 7.8.24 which calculated as 68 days between drills. The facility's fire drill log also included drill dates on 9.3.24 and again on 11.4.2024 which calculated as 62 days between drills. It was observed that the facility tested smoke detectors on 12/01/24 and then again on 1/04/25, which is greater than the allowable 30 days between testing. (34 days lapsed) Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: Fire drills will be conducted at least once per every 60 days. Smoke detectors will be tested at least once every 30 days. Inspection: Unannounced Monitoring. Staff person #1 did not complete the following required pre- service training within 90 days of their date of hire (see LIS code sheet): "Get Started with Center-Based Care-Revised 2022" and "Pediatric first aid and CPR" . The trainings were dated 96 days and 132 days after the staff person's date of hire. Staff person #3 did not complete the following required pre- service training within 90 days of their date of hire (see LIS code sheet): "Pediatric first aid and CPR" . The training was dated 91 days after the staff person's date of hire. Staff person #5 did not complete the following required pre- service training within 90 days of their date of hire (see LIS code sheet): "Pediatric first aid and CPR". The training was dated 113 days after the staff person's date of hire. Staff person #6 did not complete the following required pre- service training within 90 days of their date of hire (see LIS code sheet): "Pediatric first aid and CPR". The training was dated 119 days after the staff person's date of hire. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Correction: All pediatric first aid and CPR trainings were completed prior to the inspection , but corrections date is 2/8/25., Trainings will be completed by deadlines. Staff person #1 had completed the 'home based ' training by deadline , instead of the 'center-based' training. Staff person #1 completed the correct training as of 2/8/25 and 2/9/25. Facility self-reported that the following occurred on 2/4/2025: Staff person #1 pushed child #1 and child #1 fell to the ground. Staff #1 was immediately removed and their employment was terminated by the facility. It was reported that Child #1 was not injured and wanted to immediately go back to playing. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background.
