A renewal inspection was conducted at the facility on 2/10/26. At the time of inspection, staff person #1 only had 8.5 hours of annual training for the 2025 training year. Staff person #3 only had 11.5 hours of annual training for the 2025 training year. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A renewal inspection was conducted at the facility on 2/10/26, at the time of the inspection the facility's fire drill and detection log was reviewed and found that there were more than 30 calendar days between manual testing during the following periods: 4/18/25-5/20/25, 7/20/25-8/20/25, and 10/20/25-11/20/25. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Family child care homes must manually test its alarm at least once every 30 days and shall maintain a written record of testing with the facility's fire drill logs. 62 P.S. § 1016(c)(1) -(2). A renewal inspection was conducted at the facility on 2/10/26, during the inspection staff person #2 was observed changing a child's diaper and did not have the child wash their hands afterward. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Requirement: Emergency contact information must include the written consent signed by a parent for emergency medical care. A renewal inspection was conducted at the facility on 2/10/26. At the time of inspection, staff person #2 had lived outside of Pennsylvania in the past 5 years (New York state) and did not have NY state sex offender registry check and the state child abuse and neglect registry or proof that these had been requested. Staff Person #2 also did not have a signed disclosure statement on file. Household Member #4 has a volunteer PA State Police clearance, and their FBI clearance is not a DHS FBI clearance. Requirement: TIERED LIS: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). 1. Staff #2 must be removed from a child care position with direct contact and routine interaction with children until all missing clearances are completed and on file at the facility. Facility person/household member#4 will not be present in the home while children are in care. This portion of the plan requires an immediate correction. 2. The CPSL was revised to include that facility persons, volunteers and household members who may have resided outside of Pennsylvania in the past 5 years obtain the state(s) in which they've resided, state sex offender registry check and the state child abuse and neglect registry. Staff person #2 may not return to a childcare position at the facility until these completed documents are received by the operator. Household Member #4 may not be present in the home while children are in care until the completed clearances are received. To work or reside in a certified child care facility, the only acceptable FBI clearance must be obtained pursuant to the Department of Human Services (DHS). The operator must provide a date for when this will be completed. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #2 may not work in a child care position at the facility. Correction: I will comply with the CPSL at all time. I will make sure all clearances are review for any update and changes. Continued Non-Compliance: On 2/7/25 a renewal inspection was conducted at the facility. Staff person #1 did not have current pediatric first-aid/CPR training. This is evidenced by the previously documented pediatric first-aid/CPR training expiration date of 5/2024 . On 3/19/25 an unannounced inspection was conducted. Staff Person #1 did not have current pediatric first aid and CPR. The facility's acceptable plan of correction indicated the correction would be implemented by 4/1/25. On 4/9/25 an unannounced inspection was conducted off site. Staff person #1 still does not have current pediatric first-aid/CPR training. This is evidenced by the previously documented pediatric first-aid/CPR training expiration date of 5/2024. Requirement: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: I got her a class for April 1st. I try to get her something sooner but they didn't have nothing. She couldn't attend because she had a Family emergency that day but I got her another appointment for next week Tuesday 4/15/25 Inspection: Unannounced Monitoring.
