Observed in room 119 the first-aid kit lacked soap. Observed in room 113 the first-aid kit in the "go" bag lacked soap. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Clean and good repair (3270.102(a))

Observed in the outdoor play space a cracked lid on a toy bin. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Based on documentation of the fire detection system done by a fire professional, testing of the system occurred more than 12 months apart due to the fact the documents were dated January 2025 and April 2026. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: Contacting the fire alarm company has been added to the director's monthly checklist in March. The director will contact the inspection company in March of 2027 to make sure the inspection is scheduled to be done by the end of April 2027. Inspection: Renewal. Observed a metal framed plastic bin shelf which was not securely anchored to the wall and could be pulled over by an infant/young toddler trying to pull themselves up. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Observed in the young toddler room, a bottle of liquid on the windowsill which was not labeled with the contents of the bottle. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The bottle on the windowsill has been labeled "water" for watering the plants in the classroom. Observed a first-aid kit accessible to children on a low shelf in an unlocked shed in the preschool play area. Requirement: A first-aid kit must be inaccessible to children. Building Surfaces (3270.76)

Observed dirty air vents in the Pre-K room, young toddler room, school-age room, and the wall vent in the infant room. Observed exposed bolts on the outdoor play fence by the preschool/school-age gate and by the entrance door. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Air vents have been cleaned in pre k, young toddler and school age. The exposed bolts on the playground have been covered with bolt covers. Inspection: Allocated Unannounced Monitoring. Observed peeling covers on the two brown couches in the Pre-K room. Observed a loose screw on the end of a crib. Observed two riding toys with peeling covers and exposed foam in the toddler gross motor room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The peeling couches have been repaired with furniture tape. The screws on the crib have been tightened. The toys with peeling covers have been thrown away. Observed in the infant room, three toys which had been soiled by secretions were not immediately picked up by staff once the child was done playing with the toy and were accessible to the other children playing with toys on the floor. Requirement: Toys soiled by secretion or excretion shall be cleaned with soap and water, rinsed and sanitized before being used by a child. Correction: The staff have been reminded to pick up soiled toys more promptly to be placed in the dirty toy basket so that they are inaccessible to other children in the room after they have been soiled. The dirty toy bin has been moved to make it more convenient for staff to throw toys in it as quickly as possible. Inspection: Allocated Unannounced Monitoring. Stairs, exits, etc. unobstructed (3270.91(a))

Observed in the toddler gross motor room, the exit from the room was partially blocked due to slide and a rocker. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: The slide and the rocker were moved to another area so that there is nothing to block the exit from the gross motor room. Inspection: Renewal. On 4/1/25 observed staff file #1 and #3 had signed disclosure forms which were not signed by a witness. Observed staff file #2, who began employment after 2/1/25, contained a previous version of the disclosure statement which was not valid to use after 2/1/25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # -1, 2, and 3- may not work in a childcare position at the facility until a valid disclosure statement is obtained. Locked or inaccessible (3270.66(a))

Observed in room 115, sunscreen labeled "keep out of the reach of children", in a low cabinet with a failed child safety lock. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The child lock was replaced on that particular cupboard. The sunscreen was also moved to a higher cupboard that is less accessible to children. Inspection: Renewal. Above ground window opening restricted to 6" (3270.72(d))

Observed the second story window in the Pre-K room was not restricted to open 6 or fewer inches. Requirement: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. Correction: The tabs that keep the windows from opening were popped out on the windows so that they cannot be opened more than a few inches. Inspection: Renewal. Building Surfaces (3270.76)

Observed exposed bolts on the left gate and the corner, trash along the fence, a broken post cap with rough edges, and several cracked post caps in the toddler outdoor play area. Observed a ceiling vent which needs cleaned in young toddler room 113. Observed on the second-floor exposed drywall hole next to toilet, the sink was constantly dripping water, and several ceiling tiles were out of place. Observed several exposed bolts on the fence leading to the preschool outdoor play area. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Observed in the toddler gross motor room, peeling paint flakes on the floor by the exit door. Observed in the young toddler room, peeling paint flakes on the floor by the door. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Observed in the young toddler room 113 an unstable dress up shelf which was not securely anchored to the wall. Observed in the school age room, yellow and green stools which had loose legs and a sand table which was missing at least 5 leg support screws. Observed green and yellow chairs with loose backs in the pre k room. Observed a damaged yellow plastic chair with rough edges in the preschool outdoor play area. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The dress up shelf in room 113 was anchored to the wall with 2 wall straps, one on each end. The legs of the stools in school age were tightened. The chairs with loose legs in PreK were tightened and the broken yellow chair on the playground was removed and thrown away. Support bolts for the sand table were replaced. Inspection: Renewal. 2 feet apart (3270.106(f))

Observed in the infant room an infant sleeping in a crib which did not have 2 feet of space on three sides due to having a swing next to the crib. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: The swing in the infant room was moved further away from the crib so that it has 2 feet of space on 3 sides. Inspection: Renewal. Refrigerator (3270.107)

Observed the refrigerator in room 116 needed to be cleaned. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: The fridge in room 116 has been cleaned. Written instructions (3270.133(2))

Observed in young toddler room 113, budesonide inhaler medication which did not have a prescription or written instructions from the individual who prescribed the medicine. Requirement: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. Correction: The prescription was placed in the cabinet along with the medication and med log and will be kept there as long as we have the medication. Inspection: Renewal. Observed a bottle in room 115 which was not labeled with the child's name. Requirement: Disposable nursers and bottles shall be labeled with the child's name.
