The file for staff #1, designated as a Group Supervisor on the staff data sheet, did not contain documentation of the qualifications required for the position. The file lacked verified child care experience. Requirement: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: During the inspection, our Educational Director called the employee's former employer again and documented the employee's child care experience for the file. Inspection: Renewal. Small Toys and Objects (3270.103)

Foam paint brushes were accessible to children in Room 106 where toddlers were receiving care. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Name on bottle (3270.133(3))

Infant Tylenol observed in the infant room was not labeled for use by a single child. The medication was labeled for siblings, child #1 and child #2. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: Provider notified the family of the need to provide individual bottles of medication for each child in care. The family provided a bottle of medication for each child.
