It was observed in the toddler play space, mushrooms that could be harmful if ingested were growing at the base of the tree and accessible to children. Requirement: Toxic plants are not permitted in a child care space. Correction: All mushrooms were removed so that the children will no longer be exposed to them. Inspection: Renewal. Emergency plan (3270.27(a)(6))

The director could not provide proof of an emergency drill having been conducted annually. The last emergency drill on file to have been documented occurred 1/20/26. The director could not provide proof of an emergency drill having been conducted for the year prior to the emergency drill documented to have occurred 1/20/26. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: I located past emergency drill forms that were completed prior to 1/20/26 and emailed them to my inspector. Inspection: Renewal. Restraints prohibited (3270.113(e))

It was observed that staff person #1 was confining an infant to a small climber while outside in the toddler play space. It was stated by staff person #1 that the child was kept on the climber so that he would not get dirty or eat grass. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Correction: Staff member #1 was informed that all children must be able to walk, crawl, and explore on their own, and not be confined. Staff member #1 will never confine any child. Staff assigned to specific children (3270.113(a)(1))

It was observed that staff person #2 could not identify the specific children to which they were assigned. It was observed in the classroom that children laying on their nap cots were not able to be properly supervised by staff. The nap cots were placed in areas that caused children to be out of sight from staff. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All staff have a list of children each day that are assigned to them. They will know each child by name and their whereabouts at all times. This was reviewed with staff person #2. Furniture is now moved so that each staff member can visually observe the children at all times while they are sleeping on their nap mats/cribs. Inspection: Renewal. Supervised at all times (3270.113(a))

It was observed in the classroom, the music and sound machines being used at high volume levels that could potentially prevent staff from being able to properly hear, direct and assess children while sleeping on their nap cots. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: The music and/or sound machines are now turned down to a quieter volume so that we are able to hear, direct, and assess the children while on their nap mats. Inspection: Renewal. It was observed that nap cots being used by children were not at least 2 feet apart on three sides. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: We are now rearranging the room at nap time daily so that all nap mats/cribs are two feet apart on three sides. Inspection: Renewal. Clean and good repair (3270.102(a))

It was observed in the classroom that three of the bucket seats for the infant table were not being utilized according to manufacturer instructions as they did not contain safety straps. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: All safety straps were put into the toddler table so that it is safe and readily available for the children. Inspection: Renewal. Age appropriate (3270.101(a))

It was observed in the classroom that there is not a variety of books to promote the children's developmental needs and individual interests. There are also not enough books to prevent long waits for use. Requirement: Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use. Building Surfaces (3270.76)

It was observed that the door leading into the kitchen was open giving children access to knives placed on a table and cleaning products stored in an unlocked cabinet under the sink. It was observed in the classroom, a toy shelf that was broken at the base of the back of the shelf that could cause potential injuries to children. It was observed on the toddler outdoor play space, a tree having growth from the trunk that is sharp and pointy and low enough on the tree to cause potential injuries to children. It was observed on the toddler play space an 8 inch opening between the bottom of the fence and the ground causing potential elopement of a child. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: A gate has been placed at the kitchen entrance so that the children cannot gain access. The base of the toy shelf was repaired so that no children could not be injured. The tree was pruned, removing all growth that was sharp and pointy. Lattice was added to the bottom of the fence in the toddler play area to eliminate any openings large enough for the children to possibly elope. Inspection: Renewal. Locked or inaccessible (3270.66(a))

It was observed Magic Erasers being stored in an unsecured cabinet under the sink in the kitchen and were accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: All magic erasers are now being stored in a secured cabinet. A gate has been placed at the kitchen entrance so that children cannot gain access. Inspection: Renewal. Immunization record/ACIP recommended immunization record (3270.131(d)(5)/3270.131(e))

Upon inspection of children's files, it was found that Child #1 was missing the following immunizations: MMR, Varicella and Hepatitis A. There was no parental or medical exemption on file. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. Correction: The parent had their doctor print out + sign + date a form explaining why the child's immunizations were behind. This was done within 24 hours after is was noticed.
