During renewal inspection 4-22-26, thermometer's were not in the refrigerators in Young Preschool and Young Toddler rooms. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: Refrigerator thermometers were placed in the Younger Toddler and Younger Preschool refrigerators. Inspection: Renewal. During the renewal inspection on 4-22-26, it was observed that the outlet in the hall next to the infant room was missing protective covers. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: An outlet cover was placed on the outlet. Inpatient hospitalization or ER treatment of child (3270.20(a)(1))

During summer camp, two children were injured, each resulting in emergency medical treatment. The incidents were not reported to DHS within 24 hours of happening. Requirement: The operator shall immediately notify a child's parent and shall telephone notice to the appropriate regional office within 24 hours if a child in care at the facility is hospitalized or receives emergency room treatment. Correction: All injuries that required a child receiving emergency medical care will be reported to DHS within 24 hours. Inspection: Complaints- Legal Location. Building Surfaces (3270.76)

In classroom #1 and Classroom #2 a vent located on the wall was observed to be dusty. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Supervised at all times /Staff assigned to specific children (3270.113(a)/3270.113(a)(1))

A child was left unsupervised in Classroom 3 for approximately 15 minutes. The child was left behind when the rest of the class went to a gross motor activity. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Directed Plan of Correction: 1. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All staff will receive supervision training on 5/28/25 by ELRC staff. Staff were met with on 5/1/25 and supervision regulation was gone over. Staff #1 had an initial health assessment dated 10/31/24, which was not completed prior to initial employment. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. In classroom 3, a sensory bin was observed to contain multiple items less than an inch in diameter, which are considered choking hazards. Requirement: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care. Staff #1 completed health and safety training on 12/4/24, which was not done within 90 days of hire. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. In the mat room (gross motor room), two teachers were observed on their phone while children were in care. Upon walking in the door, both teachers quickly put their phones away. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: As of 2/19/25 a cell phone policy has been implemented. All current staff have signed they were trained and understand the policy. The cell phone policy will be reviewed with all new hires as a part of the onboarding process. ProCare is to be utilized on tablets with the exception of areas of no WIFI, such as on walks when parents pick up remotely or in cases of an emergency. Otherwise, cell phone usage is strictly prohibited while children are in care. Verbal warnings have been delivered to the staff members not in compliance with active supervision. In the Young Preschool room, a child sized blue couch near the entrance was observed to be in disrepair with exposed foam at the corners of the cushion. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: Chairs that were in disarray were thrown out. The last documented fire drill completed was dated 3/22/24. There was no updated fire drill completed since that date as evidenced by the fire drill log. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. Correction: A fire drill will be completed by end of week 10/21/24. Director will conduct a fire drill every 45-60 days.
