Letter to parents (3270.27(e))

The parent letter did not explain the facility's lockdown procedure. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. Correction: A revised parent letter has been created that includes a clear and detailed explanation of the facility's lockdown procedures. Inspection: Initial review. Building Surfaces (3270.76)

In the outdoor play space, 11 sharp metal bolts were protruding from the chain link fence. In the bathroom, 4 bowl to floor toilet bolts were not covered. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: All bolts were immediately corrected. The sharp mental bolts protruding from the chain link fence were covered with bolt covers. Glass (3270.81)

A visual strip or other visual identification was not placed on the glass double doors used to enter the facility. Requirement: A visual strip or other visual identification shall be placed on glass located in a traffic area, a child care space or a play space. Correction: A visual safety strip was immediately installed on the glass double doors used to enter/exit the facility to ensure glass is clearly visible. Emergency plan (3270.27(f))

The operator did not send a copy of the emergency plan to the local municipality and to the county emergency management agency. Requirement: The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency. Correction: A copy of the Emergency Plan was emailed to the local municipality and county emergency agency on 12-10-2025. Proof of submission has been put into our binder. Inspection: Initial review.
