Observed staff complete two diaper changes. While completing the diaper changes, the soiled diapers were not immediately discarded into a plastic-lined, hands-free covered can. The soiled diapers were placed on the changing area until the diaper change was concluded and then discarded. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. On 8/21/25, at approximately 1:30pm, CR observed Staff #3 and Staff #4 (both aides) caring for 8 children in the infant room with no other staff present. Requirement: An aide or a combination of aides shall be supervised at all times by a staff person qualified at minimum as an assistant group supervisor. Correction: Staff member #3 will be supervised at all times by at least an assistant group supervisor until all required training and hours are completed. Requirement: TIERED LIS: 1. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Correction: Staff person #1 and staff person #2 had a meeting with director on 8/22/25 regarding supervision, facility ratios, state ratios, challenging behaviors, and licensed childcare spaces. The legal entity will require all staff persons at the facility to complete a training regarding managing challenging behaviors. The training must be offered by ELRC 3, in-person and a minimum of 2 hours. The training must be completed within 3 months of the approval of this plan of correction.
