The two (2) most recent Health Assessments in the file for Child #1, a preschool child, were dated more than 12 months apart. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: The file for child #1, a preschool child, now contains their most recent health report and their file is now current. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Based on interviews with staff, a white powdery substance was seen on the floor of the Toddler 1 classroom when staff were putting away mats after rest time. It was determined by staff that a capsule containing medication prescribed by a physician was unknowingly dropped by a staff person and must have been stepped on breaking the capsule and spilling the medication on the floor. The medication was accessible to the children in the classroom. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The item that posed a threat to the health and safety of the children, was cleaned and removed immediately from the classroom. Inspection: Self-Reported Non Compliance. Based on interviews with staff, a child discovered and picked up a half of an empty medication capsule which was accessible to the children on the floor in the Toddler 1 classroom. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The item that was less than 1-inch in diameter was removed from the classroom immediately, so it would not be accessible to the children who are still placing objects in their mouths. Transcript, diploma and letters (3270.192(2)(iv))

The file for Staff #1, who had a foreign diploma/degree, did not contain evaluated proof of education, and therefore, the educational equivalency could not be determined. Requirement: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.
