On 02/24/26, during the renewal inspection, the toilets in classroom #10 and the toilets in classroom #7 did not contain signs which stated that a facility person and an able child shall wash their hands after toileting and before eating. On 02/24/26, during the renewal inspection, in classroom #11 bleach and Lysol were kept in the cabinet under the small sink, unlocked, accessible to children and the spray bottle containing bleach water in classroom #7 was kept in the unlocked cabinet under the changing table, accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. On 10/07/25, a video of staff person #1 threatening to harm a child, was captured. On 10/17/25, during an interview, staff person #1 admitted to threatening to harm a child by saying "I am going to sit on you." Requirement: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. Correction: Staff member was removed from the classroom. The child's parents and licensing agency were notified. A written report document of the incident was completed and filed. 2 hour diaper check (3270.135(e))

On 09/15/25, during the complaint investigation, the Procare reports that on 09/04/25 the diaper of child #1 was checked and changed at 9:03AM, 12:15AM, 2:50PM. On 09/04/25 the Procare reports that the diaper of child #1 was changed at 9:17AM and 12:00PM. Requirement: A staff person shall check a child's diaper at least every 2 hours and whenever the child indicates discomfort or exhibits behavior that suggests a soiled diaper. A staff person shall change a child's diaper when the diaper is soiled. Correction: All staff in diaper changing rooms received refresher training on proper diapering procedures including the required two hours minimum change schedule. Infant sleep position (3270.119)

On 06/02/25, during the unannounced monitoring, child #1 in infant room 1 was observed to be sleeping in a bouncer at 10:45am. It was reported that the child had been asleep in the bouncer since 10:12. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. Correction: Staff were immediately retrained on safe sleep practices emphasizing the importance of placing children to sleep on their backs on flat, firm surfaces. Our center's sleep policy was reinforced, emphasizing compliance with licensing and national safe sleep guidelines. Inspection: Unannounced Monitoring. Staff assigned to specific children (3270.113(a)(1))

During the unannounced inspection on 06/02/25, the staff in classroom D3/pre-k, early pre-school, PPB, preschool 2, preschool 1, toddler 2 and pre-k 2, did not know the names and whereabouts of the specific children assigned to them for supervision. When asked staff had difficulty identifying their specific group of children assigned tot hem for supervision. The staff people in the toddler 2 and pre-k 2 classrooms stated that they were not assigned responsibility for the supervision of specific children. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Management reinforced our staff assignment procedures to ensure all staff members clearly understand their designated groups before and during shifts. A supervision roster for each classroom is posed in common areas in the classroom and in the management office for reference. Inspection: Unannounced Monitoring. On 06/02/25, during the complaint investigation, it was discovered that staff person #1 was squirting children in the face with a squirt bottle while children were toileting to make them go to the bathroom and to get them used to "feeling wet". Requirement: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. Correction: Staff person #1 received a formal written warning and was terminated. On 06/02/25, during the complaint investigation, it was confirmed that after lunchtime, Child #1 was crying because they wanted their cup. Staff person #1 picked Child #1 up by their right arm in order to move child to their cot because child was upset and unwilling to walk to their cot after being directed verbally. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Staff person #1 was removed from direct care, received a formal written counseling report and then was terminated. Other staff in the classroom received immediate coaching and retraining on proper lifting and handling techniques in accordance with our policy and licensing requirements. Health and Safety Training - Pediatric First Aid and CPR (3270.31(f)(10))

At the time of the inspection, Staff 2 did not have proof of a current CPR training certificate and was past their 90 days. Requirement: Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.
