Observed a toy house with a crack at the bottom of the door frame in the outdoor play space. Observed a climber block with exposed foam in the Older Toddler Room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Mixed Age Level (3270.52)

At approximately 11:55 AM Certification Staff observed Staff #1 sitting at a table with 9 children in the Preschool section of the Front Room. When asked to name the children, Staff #1 named all 9 children. When asked to provide the ages of the children, Staff #1 identified 1 child as an Older Toddler and 8 children as Preschoolers. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Correction: There are two staff in the preschool room. Reviewed that they must both be in the room at all times because of the half walls.
