At the time of inspection, the operator did not have an updated health report for child #2. At the time of inspection, the emergency contact information for children #1 and 2 did not include the address of the individual designated by the parent to whom the child may be released. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. At the time of inspection, staff persons #1,2 and 3 did not have verification of pediatric first aid and pediatric cardiopulmonary resuscitation (cpr) certification. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: Provider will obtain verification of pediatric first aid and pediatric cardiopulmonary resuscitation (cpr) certification. Inspection: Renewal. At the time of inspection, certification representative observed several bottles and or sippy cups were not labeled with the children's names. Building Surfaces (3270.76)

At the time of inspection, certification representative observed damage to two of the dividers in the toddler room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
