    During a trip to a local park on 3/10/26, Staff #1 acknowledged that they left the park to escort one preschooler back to the facility after the child attempted to elope, leaving Staff #5 alone to supervise 12 preschoolers. There were no additional facility persons with Staff #5 at the park during this time. Requirement: At least two facility persons shall be present when children are on an excursion away from the facility. At a minimum, one of the facility persons shall be a staff person. Correction: Correction Date: 3/11/2026 It is Bloom Early Education Centers' policy that two staff must be present when staff are away from the building. At a minimum one staff must be an assistant group supervisor, lead teacher or part of the management team that meets the qualifications for group supervisor or lead teacher. Staff #1 broke this policy by returning a child to the center, she had been fully training on supervision and ration rules including the following form our New Staff orientation training. Staff without clearances, new staff training, First Aid/CPR, mandated reporter training and a certain amount of experience cannot be alone in a classroom, there always must be a senior staff member in the classroom. It is Bloom Early Education Center's Policy that ratio's are maintained at all times, both on and off the facility premises. Staff #1 broke this rule, rather than following the approved procedure of if you need help inside or outside of the building with a child staff contact someone from the management team and they will help you with the child. No other proof of education was observed in the staff person's file. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre-service trainings are completed, staff person #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. Our policy states that if a staff needs help with a child, onsite or off site one of the management team will go to help the staff. This rule has been normally followed throughout the time the center has been open.     On 3/10/26, the facility self-reported that Child #1 went missing from a local park, at approximately 10:40 A.M., on the same day. Through interviews, it was determined that on the morning of 3/10/26, Staff #1 and Staff #5 were supervising a group of 13 preschoolers together at the park. Staff #1 left the park to escort one preschooler back to the facility after the child attempted to elope, leaving Staff #5 alone with 12 preschoolers. Around the time that Staff #1 returned to the park, two children were injured on a seesaw, and both staff attended to them. Prior to this incident, Staff #5 had observed Child #1 on the swings. After providing first aid to the injured children, Staff #5 conducted a head count and discovered that Child #1 was missing. Approximately 15 minutes later, Child #1 was located at a nearby housing development in the care of residents. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. During the same inspection, Staff #2 identified 1 older toddler and 8 preschoolers in their supervision group. Staff #9 identified 7 preschoolers in their supervision group. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. The Legal Entity/Director must update existing supervision policies/procedures. The policies and procedures must include the following requirements: 1. Staff will be able to see, hear, direct and assess the children at all times. 2. Staff will know the names of the children they are specifically assigned to supervise as soon as children arrive. These assignments must maintain ratio at all times. 3. Staff will know the whereabouts of the children in their assigned group. 4. Staff will be physically present with the children in their assigned group at all times. 5. Staff will know the appropriate procedures for situations of transition, including staff lunches, bathroom breaks, and for staff to obtain items located outside of the classroom while maintaining appropriate supervision requirements. The Legal Entity/Director will submit the updated policies to the Northeast Regional Office for approval. Once approved, all existing staff and new hires must be trained on the approved policies and procedures, and it must be documented in their files. Correction: A written procedure on how to keep list of children in each classroom and each group was shared with all employees by management-they were asked if they had any questions personally. Lists will be checked by management periodically and unannounced so that we are sure that all staff are following this procedure. On both of these incidents the lists were corrected immediately.     Staff #11 acknowledged that they put diatomaceous earth under the children's carpets in the Older Toddler, Young Toddler, and Junior Preschool Rooms and behind a shelf in the Junior Preschool Room. The bag containing the diatomaceous earth was labeled "keep out of reach of children." The bag also indicated that it was to be used with "an approved respirator and goggles." Diatomaceous earth was observed to be caked onto the underside of carpets where children were in care. Staff also stated that children were present in the childcare space when Staff #11 was applying the diatomaceous earth, and no one was wearing a respirator or goggles. Additionally, an older toddler was observed to be picking up and playing with a corner of a rug in the Infant/Toddler Room. The underside of this rug was caked in diatomaceous earth, which came off the rug and onto the toddler's skin and clothing. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Cleaning materials and other toxic materials shall be used in a way that does not contaminate play surfaces, food, food preparation areas and does not constitute a hazard to the children. Correction: We immediately cleaned up the diatomaceous earth when we realized the packaging stated keep out of reach of children.
