    It was observed on 3/27/26 the toddler room has three (3) water stained ceiling tiles and two (2) ceiling tiles are missing from the toddler room bathroom. As a result, pink insulation was observed to be hanging down from the ceiling in the toddler room bathroom. It was observed on 3/27/26 the young toddler room has several water stained ceiling tiles. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Addressed via a conversation with each employee about the importance of these little things that are big things. And how an ounce of prevention can result in much safer days. I will address this again at our monthly meeting. New ceiling tiles will be going up this weekend.
