  1. High risk4/2/2026

Age and Training/Health and Safety Training - Pediatric First Aid and CPR (3270.31(e)(4)(i)/3270.31(f)(10))

Staff person #1 has completed first aid and CPR training, however the training was not completed through a Pennsylvania Quality Assurance System (PQAS) approved curriculum and the trainer has not provided documentation that they are PQAS-certified in that training curriculum. See LIS code sheet. Staff person #2 did not complete Pediatric first aid and CPR training on or before expiration of the previous certification. This is evidenced by the previously documented Pediatric first aid and CPR training on file expiring on 12/31/2025, and the most recent training taken on 3/16/2026. Requirement: Competence is the completion of training by a professional in the field of first aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Until such time as this required preservice training is completed, staff person #1 must be supervised, when interacting with children at a minimum by an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. Correction: Team Member #1 completed Pediatric First Aid/CPR training on 4/13/2026. The certificate was shared with the Certification Rep. Team Member #2 completed Pediatric First Aid/CPR training; however, the training was several months past due. Her certificate was seen by the Certification Rep. The procedure for monitoring required training will be revised to ensure 1) that team members are also taking responsibility for required training and when the training is due and 2) that we begin to sort the spreadsheet that is used to monitor required training by training dates rather than center location/place of employment. Inspection: Renewal. Source: PA DHS COMPASS provider search (compass.dhs.pa.gov).

    Locked or inaccessible (3270.66(a))

On 4/8/2025, a container of Lysol toilet bowl cleaner was seen in an unlocked cabinet underneath the sink in the bathroom that is used by the children in the Pre-K Counts 1 Classroom. The Lysol toilet bowl cleaner was labeled keep out of reach of children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: On April 8, 2025, the toilet bowl cleaner was removed. On Friday, May 2, 2025, a Team (staff) meeting was held. The Team Leader (center director), Krissy Austerberry, reviewed Child Development Inc. procedures for conducting the Daily Health and Safety Checklist required in every classroom. The importance of doing the daily check was stressed since the toilet bowl cleaner was placed in the cabinet by the contracted cleaning service. Team members signed off on the procedure review. Inspection: Renewal. Source: PA DHS COMPASS provider search (compass.dhs.pa.gov).
