A renewal inspection was conducted on 3/12/2026. At that time, facility did not have an emergency drill log on file. Operator reports that they have not conducted an emergency drill at the facility. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: We were only aware of the fire drill requirement, but not of the annual emergency drill. Annual emergency drill was conducted on 4/3/2026. A renewal inspection was conducted on 3/12/2026. At that time, the facility's fire detection testing log was reviewed. Log indicated that smoke alarms were tested past 30 days on the following occasions: 4/4/2025-5/16/2025, 5/16/2025-6/20/2025, 8/1/2025-9/2/2025, 9/2/2025-10/4/2025, 11/1/2025-12/5/2025, and 12/5/2025-1/5/2026. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Correction: The regulations 55 Pa. Code § 3290.95(a) and 55 Pa. Code § 3290.95(b) do not specify how often fire alarms must be tested. For that reason, we had been testing them monthly on different days throughout the month. A renewal inspection was conducted on 3/12/2026. At that time, there were no evacuation routes posted in the basement or on the second floor of facility. Requirement: Evacuation routes are posted in a conspicuous location on each floor of the facility. Correction: During previous inspections conducted by the City of Allentown and the fire department, we were only required to have an evacuation route map on the first floor. For that reason, there were no maps on the second floor or in the basement. This has now been corrected. We have created and posted an evacuation route map on each level of the facility, including the first floor, second floor, and basement. A renewal inspection was conducted on 3/12/2026. At that time, unlabeled bottles were observed in the facility. Requirement: Disposable nursers and bottles shall be labeled with the child's name. Correction: At the time of the inspection, we only had one child who used a bottle. Since the mother brings the bottle daily and we do not keep it overnight, it was not labeled with a name. Currently, we do not store bottles or sippy cups on-site; parents bring them and take them home every day. However, to prevent any future non-compliance, we have now implemented a policy requiring every bottle brought into the facility to be clearly labeled with the child's name, in accordance with regulation 3290.166(4). A renewal inspection was conducted on 3/12/2026. At that time, facility served chicken quesadillas for lunch. There were no fruit or vegetables served with lunch. Requirement: A lunch or dinner prepared at the facility for children of toddler age or older shall include at least one fruit or vegetable. Correction: On the day of the inspection, the children were indeed served fruit. It is our practice to serve fruit after the main course to encourage them to finish their entire meal first; however, we consistently include fruits or vegetables with every plate served. We regret that the inspector did not witness this and that the matter was not brought to our attention during the visit. Inspection: Renewal. Staff Person #1 was observed interacting with and caring for children throughout inspection. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment on file at the facility. Correction: Staff Person #1 was only present in the home during the inspection as a member of the household. She is our daughter and lives here. However, she had already completed the required health assessments; we were only waiting for the clinic to call us to pick them up. Requirement: The operator shall require the parent of an enrolled child to provide an initial health report no later than 60 days following the first day of attendance at the facility. Correction: The child's mother initially provided a New Jersey State health assessment, but our Inspector let us know that it wasn't valid for our records. We asked her for an updated version, which she has now submitted. You'll find the current health assessment attached to this email. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: During previous inspections conducted by the city, we were not required to have a thermometer inside the refrigerator, so this was a new requirement for us. Our refrigerator has a built-in digital thermometer on the door, and we believed that this was sufficient. However, we have now purchased an analog thermometer and placed it inside the refrigerator to ensure compliance with 55 Pa. Code § 3290.106. Inspection: Renewal. A renewal inspection was conducted on 3/12/2026. At that time, the cots used by children at naptime were not labeled. The operator reported that labels are peeled off by the children. Additionally, the list on the wall is outdated. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program. Correction: We have labeled each cots with permanent marker. Previously, we used name labels for each child, but they were often removed by the children. To improve this system, we have implemented a new approach. Each bed is now assigned a number (for example: 1, 2, 3, 4), and we have placed a whiteboard on the wall where each child is assigned to a specific cots number. Inspection: Renewal. Correction: We have had a first aid kit in place since the start of our operations. However, we were not aware of the requirement to include soap in the kit.
