During the renewal inspection on 1/20/26 the lower-level (basement)classroom did not have an operable fire detector. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: A new fire alarm was purchased for each classroom and installed. They are OSHA approved and proof of purchase will be maintained on file at the facility. Inspection: Renewal. During the renewal inspection on 1/20/26 the emergency contact form for child #1 did not have written parental consent for emergency medical care or administration of minor first-aid procedures by facility staff. Requirement: A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. During the renewal inspection a table was observed turned on it's side to block children from leaving the large muscle playroom to a hallway area used by a separate organization that contains a barbershop. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: The table that was turned over and being used as a barrier was immediately removed and returned to its intended use. Staff were instructed that furniture and equipment may not be repurposed or positioned in a manner that creates a safety hazard or is not approved for use as a barrier. Our landlord has begun construction (during the night) to build a wall with a door to prevent children from wandering into non-childcare spaces. Inspection: Renewal. The floor cap to the toilet used by the children in upper-level bathroom was missing. Requirement: Toilet areas and fixtures shall be cleaned daily and be in good repair. Building Surfaces (3270.76)

During the renewal inspection on 1/20/26 a partition style door in the large muscle playroom was torn and damaged. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The partition will be replaced with drywall to secure the area, and keep the area free from visible hazards. Inspection: Renewal. Building Surfaces (3270.76)

During the inspection on 9/9/25 the playground padded turf had areas where it was torn and/or damaged by slide/hill area as well as sections in the flat area surrounding the slide. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The operator had the damaged areas sanded to avoid sharp edges on 9/10/25. The children will be restricted from the slide, hill area until patches are installed and the area is completely safe for play. The patches should be completed by 10/1/25. The operator has contacted 2 contractors for repairs. We are waiting for estimates which were promised within 2 weeks. Inspection: Allocated Unannounced Monitoring. During the renewal inspection on 1/30/24 a cubbie unit was a narrow base and tipping hazard was observed in a hallway used by the children. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: Cubby was reattached after construction. Completed on 1/30/2025. Lighting (3270.78)

During the renewal inspection on 1/30/25 the lights in the interior upper-level hallway by the bathrooms were not working. Requirement: Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light. During the renewal inspection on 1/30/25 a window in the upper level "Coral" classroom opened beyond six inches. Requirement: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. No insects or rodents (3270.67(c))

During the renewal inspection on 1/30/25 mouse droppings were found in a drawer in the upper level "Coral" classroom. Requirement: Evidence of infestation of insects or rodents in the facility is not permitted. Correction: Classroom was cleaned and disinfected by professional cleaners and MAD exterminators have been notified for further assistance. Inspection: Renewal. During the renewal inspection on 1/30/25 a Pothos plant was observed in the upper-level classroom (Sunflower room). And a snake plant was observed in the hallway area. Requirement: Toxic plants are not permitted in a child care space. Correction: Plant has been removed and our center is using Google as well as plant app to identify toxic plants.
