Building Surfaces (3270.76)

At the time of inspection, a missing ceiling tile was observed in one year old classroom. Also, a damaged wall with broken plaster observed in a corner of the preschool classroom. The white floor tiles in the restroom of the PreK counts classroom were discolored with a dark gray stain which looked unclean. Also, in the PreK Counts classroom; the paint trim around the window was observed peeling. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The director will ensure all areas of disrepair cited are corrected to good repair or cleaned. The missing ceiling tile will be replaced in the one-year-old classroom. The damaged plaster on the wall in the preschool classroom will be repaired to good repair. The floor tile in the restroom of the PreK Count classroom with be cleaned to good repair or replaced with clean tile in good condition. At the time of inspection, the file for staff member #2 contained an international diploma or degree but did not include the required credential evaluation document reflecting the educational equivalency of the international education to the U.S. Department of Education standards Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: The director will assist staff member #2 with requesting an international education evaluation by directing the individuals to the Pennsylvania Keys website to apply for an evaluation. Staff members #1 and #3 will submit the completed education credential evaluation document verifying the individual has the equivalency of at least a high school diploma. Inspection: Renewal. At the time of inspection, the file for staff member #1 contained a PA child Abuse clearance indicated a pending status. The file for staff member #2 contained the wrong FBI clearance, it was completed under the Department of Justice, instead of the Department of Human Services. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility. Correction: Staff member #1, and #2 will not work in a child care position at the facility. Staff member #1 must submit the completed PA Child Abuse clearance. Staff member #2 must submit a completed DHS FBI clearance. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. At the time of inspection, the facility did not have verification of providing the parent of child #1 with a Child Service Report within the last six months indicating the child's growth and development in the context of the services being provided. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Correction: The director will ensure the child's teacher immediately completes a Child Service Report for child #1 and provides the information to the child's parent for their knowledge. Clean and good repair (3270.102(a))

At the time of inspection, in the outdoor play space a wood climber in the shape of a ship was observed with damaged peeling paint and rough surfaces. Also, observed in the outdoor play space was a wooden stump in the ground which was a tripping hazard. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The director will ensure that the wooden climber in the shape of a ship is repaired to good condition. The surface of the wooden ship will be smoothed to remove the possibility of splinters and then painted. The wood stump will be removed from the ground of the outdoor play area. Inspection: Renewal. Building Surfaces (3270.76)

At the time of inspection, damaged paint and unclean surfaces were observed in the bathroom located between the two-year-old and three-year-old classrooms. Damaged paint was observed on the wall near the classroom door in the three-year-old classroom. Also, peeling paint was observed on the inside surface of the facility's front door. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The director will ensure that the areas of the facility that were cited are cleaned and/or repaired to good condition. The shared bathroom surfaces will be cleaned, and any damaged paint will be repaired. The director will ensure that the disrepair of the damaged paint near the front door in the three-year-old classroom is repaired to good condition. The director will also ensure that the peeling paint on the inside surface of the facility's front door is repaired to good condition. Inspection: Renewal.
