At the time of the inspection facility person #5 did not have a health assessment or Mantoux test read and results on file. Requirement: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. At the time of the inspection there was not 2 feet of space on three sides around the children during naptime. In room #1 (Infant room) children # 3, 4 and 5, in room #4 Toddlers (throughout), in room #8 Pre-school I (throughout). Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. At the time of the inspection the facility was missing handwashing signs in the bathroom in room #4 Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. The row of lights in the infant room on the left side back row did not work causing the area to be poorly lit for safety and supervision of children. Requirement: Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light. Correction: Class will will keep shades on window open in crib area to let in natural light and keep lights on. Light bulb that was out was replaced. Inspection: Renewal. At the time of inspection, facility person #2, 3 and 4 did not have verified education (transcript), facility person #3 did not verified experience on file. At the time of inspection facility persons 1 and 2 did not have emergency plan training on file. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Correction: Person 1 & 2 have been trained when updated yearly. Both directors had training and they both conduct the training with me. At the time of investigation facility person#1 left child #1 outside unsupervised and unattended . Requirement: The Department may deny, refuse to renew or revoke a certificate of compliance for gross incompetence, negligence or misconduct in operating facility. Correction: Legal entity will train staff in proper face to name tracking system and a copy will be sent to OCDEL for evidence of completion. Inspection: Complaints- Legal Location. Supervised at all times (3270.113(a))

At the time of the investigation, facility person #1 left child #1 on the playground unsupervised. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: Children must be supervised at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format. Correction: Legal entity will have facility person #1 complete training in proper supervision and submit training certificate to OCDEL for evidence of supervision.
