A child and a parent were observed passing through kitchen to enter the OT room. There was a bag of Rock Salt in a bucket label " Keep out of reach of children" outside of the toddler exit door and walkway to the playground accessible to children. There was a sunscreen label " Keep out of reach of children" in a backpack hanging on the cubby in room #3 accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Clients will be reminded of our policy stating that the kitchen is a child free zone and to use the appropriate entrances to the classrooms. Paint was placed in the locked cabinet. There was fruit flies around the garbage can in the preschool room. Requirement: Evidence of infestation of insects or rodents in the facility is not permitted. Correction: To eliminate and prevent fruit flies, natural child-safe remedies have been utilized and the garbage can is scrubbed/cleaned daily and this room will take the garbage out twice a day to eliminate food in the garbage can throughout the day. The first - aid kit in the preschool room #3 lacked gloves. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Correction: Each room maintains boxes of gloves accessible for food serving and other purposes. A pair of gloves were then placed in the first aid kit for when it is taken outside. Building Surfaces (3270.76)

There were two holes on the last metal bathroom stall in Room #2. The holes had sharp edges creating potential hazard. The playground flooring outside Room #2 lacked a transition piece creating a tripping hazard. There was a hole on the infant/toddler playground. It appeared that an animal may have dug beneath the artificial grass, removing support underneath the surface and creating a shallow, uneven area. To the right side of the hole was also a raised spot that presented a tripping hazard. There was a fence net hanging off the fence creating spots where a child can get trapped. The wired fence was pulling off the fence pole in the preschool playground. There was a piece of the fence pulling off exposing sharp wired ends. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: All floors, walls, ceilings and other surfaces will be kept clean, in good repair and free from any visible hazards. In Room 2, a new toilet paper holder has been installed into the two holes from the previous holder. The transition piece to the bike path was slid over and aligned to eliminate that tripping hazard. More support is added into the shallow area under the grass to create a more even ground while still providing accurate drainage flow underneath. The fence was tightened to the pole and trim while the privacy shade connected to the fence to defer a child from scaling the fence as an accommodation has been removed. Inspection: Renewal. There were a broken green toy and a deflated yellow in the preschool playground. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The cracked bin in the Older Toddler (Green) Room was removed, the broken divider and cubby in Room 2 were repaired along with the broken white lid to the plastic drawer in the teachers' closet in this room. The file for staff #5 lacked a TB test. The file contained a TB from the collage dated 9/1/22. Requirement: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. Correction: Staff #5 had completed a tuberculosis screening prior to employment through her college for student teaching but was not completed on the Staff Health Assessment required by the Department and is no longer working at the facility. On 2/26/26 at approximately 12:40pm, in the older toddler room upstairs, staff identified 8 of 9 children laying on their mats. Staff #2 identified 2 OT and 2 YT in their supervision group. Staff #3 identified 1 OT and 3 YT in their supervision group. Neither staff identified Child #1 in either of their supervision groups. Child #1 was identified when Staff #4 named all the children in the group and assigned Child #1 to Staff #3's supervision group. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff members are assigned specific students to their groups. Student name tags are used to assign students to a staff member. Teachers are required to count their tags, count their students, and count their tags again and then check with any and all co-teachers in the room to make sure each child is in the responsibility of one of the teachers before responding to the question of who is in your care ensuring that they take their time to correctly answer and ensure each child is accounted for and not claimed by more than one teacher. White boards will be provided for all classrooms and recommended for teachers to write the names of each child in their group as a second way to ensure that they state all children in their assigned group. On 2/26/26 in the infant room at approximately 2:45 pm, an infant was observed in a state of high-intensity cry. The infant's face was flushed, eyes were tightly closed, and the body exhibited significant muscle tension while positioned in an infant activity table/seat. This level of crying had been maintained at a consistent high volume and intensity for a minimum of 10 minutes. During this interval, Staff #1 was present in the room watering plants with their back turned toward the infant. When asked why no attempt was being made to console the infant, Staff #1 stated, "Babies cry." No soothing interventions were observed by Staff #1 during this period. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: Prior to enrollment families are provided opportunities to discuss their parental philosophy with the classroom teachers to ensure they are fostering the parents' desires and raising the child according to the parents' wishes. If a parent requests caregivers to foster independence by reducing constant holding and following boundaries set by parents. Teachers must find ways to calm and soothe upset children without constant holding such as utilizing daily activities that promote the development of skills, social competence and self-esteem. These daily activities are to be individualized providing children with a variety of choices that are age and developmentally appropriate. If attempts have failed and the student continues to signify "purple crying," staff members are to utilize the tools learned to calm themselves, such as placing the child in a safe space, breathing, counting and/or turning on relaxing music before returning to attempt to settle the child including calling for help from a co-worker. In this classroom there are no plants, so the staff was not watering plants during the time this observation was made. However, when the child utilized all their distractions, the staff member should have requested assistance. A documented discussion was had with the employee and discipline actions were taken.
