At approximately 9:30am, Children #1, #2, #3 and #4 were observed in the hallway with 3 therapists unrelated to the Legal Entity. A Staff person was not physically present with the children during this time. Written parental consents were not present giving permission for the children to be unsupervised with the therapists. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. At approximately 9:30am, Children #1, #2, #3 and #4 were observed in the hallway with 3 therapists unrelated to the Legal Entity. In Classroom 208, Staff Person #1 was observed to dispose of a soiled diaper by lifting the lid with their hand. The hands-free mechanism was not used to allow for appropriate disposal of the soiled diaper. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Clean and good repair (3270.102(a))

In Classroom 206, one basket style bin was observed to be broken in a couple areas which caused potential pinch points. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. In Classroom 204, one electrical outlet was observed to be missing a protective receptacle cover. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. General Health and Safety (3270.21)

A loose crib sheet was observed on one of the cribs an infant was observed to be resting on in Classroom 208. The crib sheet provided by the parent was for a standard size crib, as opposed to the smaller crib present, resulting in the sheet not fitting properly to allow for the required tight fit. Training regarding plan (3270.27(c))

The updates to the emergency plan training for Staff Person #2 exceeded 12 months between trainings dated 2.5.24 and 6.13.25. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. The file for Staff Person #3 lacked completion of annual fire safety training within 12 months of starting at the facility. The fire safety training was completed 450 days after starting at the facility. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Two packages of Lysol wipes and one bottle of Adhesive Remover were observed in an unlocked cabinet near the entrance of Classroom 206. A "PocketBac" hand sanitizer was observed in the front pocket of a bookbag in a child's cubby. The Lysol wipes, Adhesive Remover and hand sanitizer could be accessible to school age children receiving care in the child care space. All items were labeled Keep Out of Reach of Children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. The bookbag containing the first aid kit was observed to be hanging from a hook in Classroom 206. It was observed that some of the school age children receiving care in the space could reach the bookbag, which made the first aid kit accessible. In the downstairs Gross Motor Room peeling paint was observed on the wall to the right of the entrance to the space. In the stairwell leading to the Gross Motor Room damaged plaster was observed on the wall above the stairs resulting in pieces of plaster to have fallen onto the stairs. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: maintenance will repair the plaster Inspection: Renewal. A basketball hoop was observed in front of the door in the Indoor Gross Motor Space. This resulted in the egress being obstructed. One cracked bin, with two potential pinch points, was observed on a shelf accessible to children in Classroom 206. In the downstairs Gross Motor Room the coating on two foam blocks and a green ball were observed to be peeling off causing the foam to be exposed. The space is to be utilized by all age groups. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. In Classroom 202, 14 preschoolers were observed to be receiving care. The mat chart in place only identified 8 assigned mats. Staff reported that all the children are present for naptime. This resulted in rest equipment not being labeled for use of a specific child for 6 of the children present. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. In Classroom 202, two children were observed to be sleeping on rest equipment that had been spaced at least 2 feet apart, but a large pillow was observed between the rest equipment. This did not allow for free space to be present as intended.
