The five windows in the upstairs hallway outside the school-age classroom accessible to children did not have the mechanisms activated that limit the opening to 6 or fewer inches. On the wooden ramp leading to the outdoor play space the wood was observed to have cracks in one of the boards causing a visible hazard. On the wooden fence by the ramp one of the wooden boards was broken off and had an exposed nail and wood causing a visible hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Then staff person #1 grabbed the child's wrist again, lifted the child off of the bike from the child's wrist, and sat him down very roughly onto the ground.
