Observed 3/25/2026 in pre-kindergarten there was baby wipes labeled "keep out the reach out of children" located in on the children's table and on the sink where children wash their hands, the baby wipes were not locked or made inaccessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: All baby wipes were removed immediately. Name/address/phone release person (3270.124(b)(7))

Observed 3/25/2026 child # 1 file was missing the addresses of the individuals designated by the parent to whom the child may be released to on the emergency contact form. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Toddler/preschool: updated health report every 12 months/Initial and subsequent health reports (3270.131(b)(2)/3270.182(1))

Observed 3/25/2026 child #2 was missing an updated health report on file, the current health report on file was dated 3/20/2025. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: Child #2 physical was completed 3/23/26 but did not give it to us until 3/26/2026. We have it on file now. Building Surfaces (3270.76)

Observed 3/20/2025 in Classroom 3 Preschool Classroom the toilet cap in the bathroom was dislodged from the floor and the toilet. In classroom 4 Toddler B the sink was leaking water underneath where children handwash after toileting In the PreK 2 classroom the wall was damaged near the children's table and classroom space. In the basement for school age the bathroom tile was damaged where it was not in good repair and free from visible hazards. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The preschool classroom toilet cap was fixed, and the cap was replaced. The sink was fixed and no longer leaking water in Toddler B. The wall was spackled and painted in Pre-K.
