The facility does have a menu that operates on a two-week rotating schedule. None of the staff present on this day could identify which week they were on. Per the Director, this menu is typically followed; however, if they are out of items they will substitute with whatever they have. This does occur frequently. Staff corroborated the fact that this happens frequently. Requirement: An operator shall conspicuously post the menu at least 1 week in advance or provide a menu to each family. Correction: All workers present were made aware of the current week and informed as to the meals available for the current day. The menus were already posted conspicuously. In the main hallway, a bottle of Family Guard Disinfectant Spray, marked Keep Out of Reach of Children, was observed to be left out on a desk in reach of children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Child #2 was observed sleeping in a crib which lacked 2 feet of space on 3 sides. One side was obstructed by another crib approximately 12 inches away. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: The crib Child #2 was sleeping in was moved so that 2 feet of space was on 3 sides. The side that was obstructed by another crib was cleared by moving the cribs so that 2 feet of space was on 3 sides. Inspection: Unannounced Monitoring. Infant sleep position (3270.119)

Child #1, an infant, was observed sleeping in an inclined seat which was placed on a raised mattress inside a portable crib. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. Correction: The inclined seat Child #1, an infant, was sleeping in was removed from the raised mattress and relocated outside the portable crib. Child #1 was transferred directly to a crib and placed in the sleeping position recommended by the American Academy of Pediatrics. Inspection: Unannounced Monitoring. Until such time as the required pre-service trainings are completed, staff persons #1, #2, and #3 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. If there are no staff person(s) available to supervise staff persons #1, #2, and #3 staff persons #1, #2, and #3 may not work in a child-care position at the facility. The files for staff #1, #2, and #7 contained documentation of completing emergency plan training more than 12 months apart, in October of 2024 and again in December 2025. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. The file for staff #2 does not include acceptable fire safety training documentation. The certificate on file dated 2/25/25 is not a preapproved course and does not list the topics covered by the training. The files for staff #1 and #7 contained documentation of consecutive fire safety trainings completed more than 12 months apart but are now current. The trainings for both staff were dated 9/15/24 and 1/05/26. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. The file for staff #6 contained an expired NSOR dated 9/22/20. Staff #6 was observed caring for children unsupervised. Staff #6 may not work in a child care position at the facility. The file for staff #4 contained an FBI clearance which was not updated by the expiration of the previous clearance. The previous FBI clearance was dated 10/05/20 and was renewed on 12/22/25. The file for staff #7 contained 3 clearances which were not renewed by the expiration of the previous clearances. The dates were as follows; previous child abuse dated 9/25/20 and renewed on 10/09/25, previous FBI dated 9/20/20 and renewed on 9/26/25, previous NSOR dated 9/22/20 and renewed on 9/26/25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. A soiled pull up was observed in an open top trash can in the PS restroom beside the sink. Used food service items and left over food were observed in an open top trash can in room 5. Lids for the trash cans were present, but not attached or in use. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Above ground window opening restricted to 6" (3270.72(d))

Second floor windows in Room 4 and Room 5 were not restricted to open 6 inches or less. The windows were accessible to the preschool and school age children who receive care in the rooms. Requirement: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. Crumbling plaster was also observed on the floor under the damaged area.
