In Room #2 (Infants) in the back left hand corner of the room, a changing table pad was ripped with exposed stuffing that posed a choking hazard to the children in the room who are still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. A trash receptacle in Room #3 (Pre School) containing human secretions (dirty plates with food, dirty napkins, used cups with juice still in the bottom) was not contained in a closed, plastic-lined receptacle. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Based on a conversation with the director, the facility did not have an annual traffic letter nor was it sent to the local authorities. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. On September 18, 2025, the Western Regional Office of the Office of Child Development and Early Learning (OCDEL) was notified by the Bureau of Workers' Compensation Compliance Department that the legal entity failed to provide the required documentation of current Workers' Compensation Insurance. Requirement: 1. A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. This operator shall provide a date when compliance will occur. 2. The legal entity shall complete the steps as provided by the Bureau of Workers' Compensation Compliance Department to verify workers' compensation insurance. This operator shall provide a date when compliance will occur. Correction: Provider got new Worker's Comp information from Traveler's Insurance.
