During a complaint investigation on 4/15/2026, it was found that on 2/26/2026, and 3/4/2026 an incident report was not given to the parent who picked up the child #1 on the day of the incident. During a complaint investigation on 4/9/2026, it was found that staff #1 used harsh and demeaning language towards child #1 in the hallway near the office. During the complaint investigation on 4/9/2026, It was found that staff person # 1 singled out child # 1 when providing the children in room #4, a candy (lollipop) excluding child # 1. Requirement: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. During the complaint investigation on 4/9/2026, it was found via video footage reviewed, that staff person # 1 pulled child #1 up from the floor by one arm. Requirement: A facility person may not use any form of physical punishment, including spanking a child. During the renewal inspection on 3/5/2026, the files for staff #1was viewed and the file for staff #1 had a National Sex Offender Registry Clearance that was date 3/1/21 which is over 60 months old. During the renewal inspection on 3/5/2026, it was observed in Room #8 on the changing table pad were Aquaphor Cream and Clorox Wipes and they were accessible to the children. During the renewal inspection on 3/5/2026, it was observed in room #8 that the changing pad had a tear in the cover, the play couch in room #1 the cover was peeling, and in room #6 there was a soiled bookcase with a green liquid stain on it. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. During the renewal inspection on 3/5/2026, it was observed in Room #1 that upon request, staff #2 could not identify the children in their primary care group. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. During an unannounced inspection on 12/30/2025, it was observed that the front exit door was obstructed with a chair. During an unannounced inspection on 12/30/2025, it was observed that the floor tiles in the front corner were breaking apart. During the complaint inspection on 4/14/2025, it was found that, staff #1, left child #1 unattended for a 30 second timespan while the child was on an enclosed playground. Certification Representative viewed the 30 second timespan on video. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. During the renewal inspection on 2/27/2025, it was observed that staff #1 did not sanitize and wipe down the changing table after changing the diaper of child #8. During the renewal inspection on 2/27/2025, the medication expiration dates were viewed for child #1, child #2, and child #3 and the Epipen for child #1 expired 3/2021 and Benedryl 9/2024, the Albuterol for child #2 expired 11/2024, and the Epipen for child #3 expired on 4/2022.
