During the initial inspection conducted on 12/5/2025 observed that the legal entity did not have a current copy of the insurance policy on file at the facility. Requirement: The legal entity shall have comprehensive general liability insurance to cover all persons who are on the premises. A current copy of the insurance policy shall be on file at the facility. Correction: I purchased liability insurance, but the company has not given me the paperwork. They were called and it was requested in front of the certification representative. I will send a copy once I receive it. Inspection: Initial review.
