During the renewal inspection on 3/2/2026, it was observed that the outdoor play space equipment were soiled and needed to be cleaned. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. During the renewal inspection on 10/2/2025, the file for staff #1 was viewed and the file contains a Mandated Reporter Training dated 9/30/20, a Child Abuse Clearance dated 7/14/20, and a National Sex Offender Registry Clearance dated 7/8/20 which is more than 60 months old (Hire date see code sheet). Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. During an Allocated Unannounced Inspection on 3/19/2025, it was observed that there was a child's play chair that was in front of the back door blocking the exit. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: Will move the chair away from the door. During an Allocated Unannounced Inspection on 3/19/2025, it was observed that there were tree branches and leaves in the back yard and overturned play equipment that were a safety hazard for the children. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.
